Short-Term Stay After Hospital Admission
Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

In-Patient Residential Stay
Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

Specialty Programs

[review] => I was in Macclenny Nursing and Rehab with 2 broken legs. They took me in and got me on my feet again! I suggest that if anyone is in need of therapy, go to MNRC. I'm able to go home tomorrow because of the care and rehab I received here. If I ever need therapy again, MNRC will always be my first choice. They cared for my husband in the past as well. [review] => DO NOT allow anyone you love or care about to stay in this facility. They only care about getting your money in the door. The rooms aren't cleaned properly, medications are not given correctly, requests for medical care are put off or denied for weeks or months. There are roaches in the rooms and the staff cover up each others mistakes or straight NEGLECT of its patients. This is a true reviw and will not even try to be debunked by this facility, as they know there is proof and photos to back up these claims. I just do not want to see anyones loved one on the news because they passed away. If you can not check on your loved one daily, I urge you to please skip past this place!!

[review] => I am a former Unit Medical Speciailist for the Geriatric Unit of NEFSH. I am currently a resident of Macclenny Rehab due a severe spinal injury that requires a great deal of physical care which I am given with professionalism, kindness, dignity and respect. All members of the staff work physically and mentally taxing positions going above and beyond to care for their patients as if they were family. They have one of the most demanding jobs you can have, and one not many could do.

[review] => Go see about your family if they are here. Otherwise the care will lack. We missed a few months visiting and calling. We visited without notice. Our loved one looked bad. His hair was matted, he smelled of urine, his oral care had not been done and his skin was dry and cracked. I take pictures every time we go and looking back at old pictures you can easily see the difference. They acknowledged the way he looked, making lots of excuses and promises to get it right. We shall see.

After my mom made it to the new facility the administrator of the new location called me and told me that Macclenny Nursing and Rehab had put my mom in a taxi cab and sent her to the new facility. My mom having Alzheimer’s didn’t know or understand what was going on and she was non verbal and the taxi cab driver had told the new facility that my mom was hitting him and tried to bite him while he was trying to drive her to the new place.
It is some sorry people who would stoop that low to have a patient that was non verbal and had full blown Alzheimer’s transported by taxi cab.

[review] => My father passed away from being at this aweful facilty. The staff is very unattentive and they do not check up on their patients. My father fell out of his bed and laid there for over 33 minutes. His roommate kept calling for the staff but nobody ever came. 911 had to be called. Also, during his stay my sister walked into the bathroom to find a dead elderly woman sitting on the toilet. They do not document all the medication that is given to the patient. This place is a disgrace and needs to be shut down!! They do not care about your loved ones. My dad had brusies on his arms from being strapped down to his bed and he was severly dehydrated!! STAY AWAY FROM THIS PLACE!!!!!!!

The higher-ups involve are extremely bad at their job. Them constantly firing people and then expecting others to come in for a job isn't a good for the home and I'm surprised there's actually people still making an effort to work there with the abysmal treatment to both residents and the staff that actually do their job. My husband Johnny agrees that this place is problematic and needs fixing up to do or it'll fall apart. If you're looking for a home to go to yourself or to send a family member, DO NOT COME HERE. It is very obvious that they care about their residents and their well being. The staff takes time to sit and talk with my grandmother as well as feed her since she has become unable to do so for herself. [review] => I would never allow a loved one to visit this facility, people you care about should not be made to suffer. If medical attention is needed, there must be an alternative compared to the traumatic experience that those close to me had to survive. Their patient was transported there in the evening and then Life Flighted out 8 hours later. The nighttime staff was unorganized, inattentive and clearly did not care about anything more than payment for showing up to work. This may be the only negative review but as of today only 17 positive reviews have been posted, only 17 people have been impressed enough give their opinion.

[review] => This place was awful. They didn’t not care for my mother properly. She was only supposed to go for a short time to recover from having tracheotomy and a feeding tube put in .. they almost killed my mother. The staff was rude an incompetent. If anyone you care about is in this facility get them out ASAP

[review] => I've been at this facility since January 18, 2019 for rehabilitation after surgery. Since arrival, I have been treating with the most professional, caring, and respectful manners from the entire staff here. The nursing, CNA's, activities, laundry, and the kitchen do a great job and their best to accommodate any needs or request. My most amazing experience here has been the therapy department including Physical, Occupational, and Speech. I am very grateful for the rehab and treatment I am receiving and if I had to pick a facility in the future it would definitely be Macclenny Nursing and Rehab.

[review] => It appears that some of the staff at this facility have their priorities out of order.The financial office being this department.We have a family member is need of a medical procedure but it can not be done until the needed paper work is completed so the funds can be distributed for payment while our family member waits in pain.Without the requested documentation treatment can not be given.Nobody calls with an update & then when I call the staff member has left early.If they the person in need of this procedure I assure you they would find it a Priority but since it does NOT affect their well being it is on the back burner it appears? It's very selfish on the part of those involved & very Unprofessional as well.

[review] => For some reason they had issues with my mom, who was brought in with Dementia. They drugged her to the point of being in coma state for seven days and was transported to ER because of being unresponsive. They tried every was possible from day one to send her to another home and or Psych ward. My mom came in facility able to walk and make her on soup. She left barely able to walk after less than a month. The food was not very good. The facility was nice and the CNA's were nice and very helpful.

Rebab staff are excellent, especially Bruce and Erica. All staff were friendly. The APN was especially helpful. Would definitely recommend this facility for rehab care.
